From applications for direct Eagle Mount participants to applications for family members of those participants, you can find it here. We require anybody that engages with Eagle Mount to fill out an application in order to collect contact and emergency information.

If you are a person with a disability or a young person impacted by cancer, begin your journey with Eagle Mount here. Fill out this application, and members of our staff will reach out to you shortly.

Family Application

​Eagle Mount supports family members of people with disability and young people impacted by cancer. As a family member, you are also eligible to participate in select Eagle Mount programs, including Big Sky Kids, Family Engagement, and our Family Swim program. If you are interested in participating, begin the process by filling out this application.

Please note: This application must be filled out for EACH family member planning to participate.

Waivers

​Unfortunately, waivers are a modern day requirement, and engaging with Eagle Mount is no exception. We require various waivers, liability releases, and assumption of risk forms to be filled out for anybody that engages with Eagle Mount as participant or volunteer. You can sign these forms at your convenience. Please only sign the ones applicable to you.
